National Health Care for the Homeless Council: National Resource Center (NRC) creates an online resource repository to meet the information needs of professionals for the purpose of building stronger and healthier communities. The Health Center Resource Clearinghouse addresses the demands placed on a busy public health workforce by providing resources and tools to acquire and use targeted information on a daily basis.

Oklahoma Advance Directive for Healthcare: Printabe PDF document.
Durable Power of Attorney: Allows a principal to appoint an agent to handle their financial affairs. The form remains valid if the principal should become incapacitated.
General Power of Attorney: Similar to the “durable” form in that it allows a person to appoint another to handle general financial affairs, but this type terminates upon a person’s incapacity.
Limited Power of Attorney: Used when a person wants to appoint someone to handle a discreet transaction or act for a short time frame.
Oklahoma Durable Medical Power of Attorney: Used when an individual chooses to appoint a trusted person to handle their health care decisions in the event that they are no longer able to do so.
Oklahoma Power of Attorney Recovation Form: This form is used to revoke previous POAs. All relevant parties, including your agents and successor agents, must be informed.

City Lights: City Lights Foundation is a nonprofit organization that has been bringing hope and dignity to those experiencing homelessness since 2013 through relational service and dignified housing.
WHAT WE DO: City Lights operates several programs in the Tulsa area that serve individuals experiencing homelessness.
WHY WE DO IT: We believe in the power of meaningful relationships to restore community. We focus on serving our most vulnerable neighbors- connecting human to human, heart to heart.
Housing Solutions: Housing Solutions’ mission is to lead in the development, advocacy and coordination of community strategies to prevent and end homelessness. We believe that together, we can create and sustain a community where homelessness is rare, brief, and non-recurring through strong public/private partnerships, driving collaboration among stakeholders, using data to guide our strategy, and educating the public on homelessness and its solutions.
A Way Home for Tulsa (AWH4T): is a collective impact of over 30 voting organizations that exists to plan and implement strategies that support a system of outreach, engagement, assessment, prevention and evaluation for those experiencing homelessness, or those persons at risk of homelessness, within Tulsa City/County.
